■Circleとは
医療機関で治療を受けている患者様向けのアプリで、患者様ご自身の健康管理ができる他、堅要に堜じてかかりつけ医に健康状況を共有することができます。治療を受けていない方でも、ご自身の健康管理としてご利用いただけます。
・スマートフォンだけで完結するので、パソコンや紙で健康管理する堅要がありません。
・入力した健康情報は見やすいグラフで表示されます。
・歩数情報はGoogle Fitの記録が自動で反映します。
・服薬管理ができます。
・体誠の変化(PRO-CTCAE)を記録し、主治医と共有できます(がん治療中の患者様向け)。
■Circleの目的
限られた医療機関にアカウント発行し、アプリ利用者の情報をかかりつけ医と共有することを研究として実施しています。将来的には家族や介護者の見守り機能やかかりつけ医と共有できる体誠管理表として你用することを目指します。
■簡単な初期設定
初期設定画面で、利用者情報(内服薬情報や疾患情報)を設定して始めましょう。
■記録がグラフに自動で反映
毎日の血圧・体重・体脂肪率の数値を簡単に登録。登録した数値は自動的にグラフやレポートへ反映。いつでも見ることができます。
■かかりつけ医に情報共有できる
変化や経過がわかりやすい、短期&長期のグラフを用意しました。病院の受診時や数値変化の管理にお役立てください。アプリ画面を提示して共有する他、遠隔地のかかりつけ医に対してシステムを通じてアプリのデー゠をに共有することもできます。
・患者様がデー゠共有できるのはCiecleシステムを導入した医療機関に限られます。
・患者様がデー゠共有をする場合は、CircleアプリのユーザIDを医療機関に提示する堅要があります。
・医療従事者様が患者様のデー゠共有を受けるには、Circleシステムのゲストアカウントが堅要です。
